The work can be challenging,

Due to staff changes within
the team, we now have a Band 6 vacancy for a nurse, Occupational Therapist or
Social Worker. This is a full time post for an experienced practitioner to have a fantastic opportunity to
develop their skills as a care coordinator with scope to be imaginative, creative
and flexible in your practice under the EIP service framework. There is lots of

Main duties of the job
If successful, you will be a Care Coordinator, working within the CPA
framework. You will work with a capped
caseload of approx. 15 service users.
You will develop
and perform assessments and risk assessments for people experiencing symptoms
of First Episode Psychosis or At Risk Mental State (ARMS).
You will work in
collaboration with the service user and their carers in offering a meaningful
care and treatment plan.
You will also
work in collaboration with your colleagues, supporting them with their
caseload. They, in turn, will support
you with yours.
You will play a
key part in developing Carer support and education programmes.
You will
participate in social inclusion groups with service users and other members of
the team.
You will support
our talking therapists in preparing service users for Cognitive Behaviour
Therapy, and participate in Family Interventions for psychosis.
You will develop
and enhance your knowledge of psychosis, ARMS, and co-morbid conditions.
You will develop
your teaching and supervision skills with learners.

Works within a multi-disciplinary team as a member of a Core
CMHT.
Management
of own caseload in line with C.P.A. procedures and risk management protocols.
