An opportunity has arisen
within Kirklees Older People’s Mental Health Services which is a service model
based on delivering ‘Care Closer to Home.’
The
Kirklees Older People’s Services Community Mental Health Team is a
multi-disciplinary team working with people who experience a functional and/or
organic mental health diagnosis.
We are looking to recruit Band
6 Community Mental Health Practitioner (Registered Mental Health Nurse) to play
a key clinical role in delivering a high standard of care.
The post is for 22.5
hours per week, Monday to Friday working 9.00am - 5.00pm and is based at Folly
Hall, Huddersfield, where case management will be held. However, as the team
has two geographical bases (Folly Hall Huddersfield and Beckside Court Batley)
as a team we are required to deliver a mental health service Kirklees wide in line
with clinical demand.
We
are looking for an individual who is enthusiastic about working in an
innovative, flexible, and creative way. You need to be committed to working
collaboratively with service users, carers, and other disciplines to develop a
plan of care and recovery.
Part of the offer includes an
induction period, regular caseload, and management supervision to promote
career progression and professional/personal wellbeing.
Main duties of the job
The successful practitioner
will work within the remit of the Band 6 job description.
Working within the CMHT, the
role will be working
with service users with both short-term interventions and severe and enduring
mental health needs. It is desirable you are proficient in undertaking
assessments of service users, deliver short term intervention work, care
coordinate, manage a case load and be able to demonstrate clinical decision-making
skills whilst working collaboratively with the service user/carers, partner
organisations and primary care staff.
A key element of the role will
be care coordination and leadership, and the ability to support colleagues with
day-to-day caseload management, duty worker and including prioritisation of
responsibilities in times of high clinical demand.

Ensures safe carriage, storage and administration of drugs prescribed (registered nurses only).
Responsible for the ordering of stock with due economy.
Produces monthly statistics in line with trust directives.
Keeps the Management Team and colleagues informed of developments relevant to the area of responsibility.
Education:
Supervises students and trainees on placement.
Assists in the teaching and in-service training of staff.
Is conversant with current trends in clinical practice.
Participates and accepts delegated responsibility in research and clinical audit as required.
Attends courses or training sessions relevant to the updating of knowledge and experience.
