Job summary
We
have a fantastic opportunity for an enthusiastic and motivated registered or
soon to be registered Pharmacy Technician to join our forward-thinking team
working within our medicines optimisation pharmacy service.
We are
looking for a highly motivated, committed & dedicated individual who is a
strong team player with excellent time management, interpersonal, & organisational
skills. You must be able to work well under pressure, have strong attention to
detail and be an excellent communicator.
As a
department our key focus areas for development are aimed at our medicines
optimisation and sustainability agenda.
The postholder will play a pivotal role in supporting us to achieve
these priorities through completing drug histories, consulting with service
users and carers regarding medicines, leave and discharge planning, utilising
near patient dispensing and liaising with primary care to enable smooth
transition post discharge.

Main duties of the job
The
successful candidate will work with the speciality pharmacists and all members
of the MDT to co-ordinate all aspects of the patients medicines related
journey, communicating effectively to ensure a safe, seamless and integrated
transfer of care for our patients.
Key
result areas include,
Supporting
clinical teams with the supply, storage and management of medicines within
inpatient areas.
Complete
medicines history/reconciliation when patients are admitted into our care.
Provide
information to patients and carers about their medicines and support adherence
and understanding.
Work
closely with clinical teams to plan and support timely leave and discharge,
utilising near patient dispensing process to ensure medicines are received in a
timely manner.
Support
with transfer of care into primary care, utilising the DMS service and liaising
with other healthcare professionals.
Participate
in the accuracy checking rota within the dispensary.
Support
the training and development of trainees.
Participate
in the weekend and bank holiday rota.
