JOB SUMMARY:

This job description is an outline of the main duties of the post. The postholder will be required to undertake other duties commensurate with the grade as directed.

This post will contribute to tackling inequalities in health through promoting and supporting people to stop smoking. It focuses on engaging with people in communities and offering them practical support to change their behaviour to achieve their own choices and goals. The post holder will provide smoking cessation advice and support to individuals and groups of people who want to stop smoking.

Specifically the post holder will:

Seek out opportunities to develop and implement stop smoking services in a range of health and community settings.

Promote the general health and wellbeing of people who use the service through offering healthy lifestyle advice and support.

work with people to change their behaviour to improve their health; and Organisational Chart: * Structure may differ slightly between service areas.

KEY RESULT AREAS:

The postholder will:

1.1 Care/service/function provision: eg,** To deliver excellent levels of customer service to people, and address or report, in a timely and proactive manner, where these standards have not been met*.

To plan and deliver smoking cessation advice and support sessions, including advice on stop smoking medications, working within recommended protocols and guidelines. This will involve individual, telephonic, on-line and group work sessions in a variety of settings and venues, working closely with the central Yorkshire Smokefree telephonic Hub staff as required.

Develop individually tailored support plans including recommending treatments based on assessed needs.

Provide information to people about stopping smoking and health-related issues in line with best guidance and protocols.

Provide general health and lifestyle advice and support to people using the service e.g. healthy eating, physical activity, stress management. This will include individual and group based interventions, signposting and referring to other health, community and voluntary services.

Responsible for the delivery of stop smoking support to specific client groups (e.g pregnant women, routine and manual workers)

Deliver very brief, brief and awareness training in stopping smoking as required.

To be involved in practice awareness raising sessions in Primary Care, Secondary Care and Community settings.

To be involved in the role out of Quit Manager training for internal staff and advisors within Primary Care, Secondary Care and Community settings.

To be involved in monitoring and evaluation of attendance at smokefree support sessions and contribute to the completion of the minimum data set requirement. This will include following up quitters who do not attend for planned support.

To be involved in the marketing and promotion of the service alongside other colleagues. This may involve attending events on evenings/weekends.

Undertake specific project and/or development activities in conjunction with the Service Manager and Specialist Advisors.

Maintain accurate client electronic records, using Quit Manager, in line with Trust guidelines.

Respond effectively to referrals within stated timescales self-referrals from the individuals, and those from colleagues /other workers/GPs.

Ensure administrative duties are carried out efficiently and effectively in order to meet the needs of the service.

To be involved in monitoing and evaluation of client feedback, offering recommendations as part of the team to improve services.

1.2 Working as part of a Team: eg, ** As part of a team, deliver person centred care whilst meeting the service standards and objectives.

Alert specialist advisors or line manager to any issues in work (including concerns about individuals or work in the community) and maintain confidentiality.

Actively contribute to ways in which local services can be improved, supporting improvements in the health and well-being of the local community and barriers that individuals are experiencing in changing their behaviour.

Contribute to the development of the Smokefree service targets and any plans that may be developed by attending meetings as appropriate with co-workers and sharing information.

To maintain good working relationships with all levels of staff.

To work closely with other team members and health promotion colleagues across South West Yorkshire Partnership NHS Foundation Trust and the wider community.

To liaise with others in the health and social care community in order to promote the stop smoking service and encourage appropriate referrals.

1.3 Managing Self: eg, ** Participate in clinical supervision to develop skills and knowledge of clinical care and practice and apply continuous reflection on own practice).

Plan own time and activities to meet the reasonable needs of individuals in the community

Take an active part in developing own knowledge and skills.

Seek advice and support as and when appropriate.

Monitor and maintain health, safety and security of self and others.

Participate in an annual performance and personal development review.

Actively participate in clinical supervision.

1.4 Management Supervision and Support:

The post holder has no managerial supervision responsibility but is expected to be a good example to other colleagues and new staff starting within the service.

1.5 Financial Responsibility:

Use Trust resources efficiently and suggest efficiency savings to more senior staff.

Person Specification

Experience

Essential

• Experience of providing help, support and/or counselling on a one to one or group basis.
• Ability to work and communicate in group settings.
• Experience of providing healthcare, healthcare advice or health promotion advice to patients or members of the public.
• Significant experience in community development work or in working with people to change their behaviour.
• Experience of using Microsoft Office software ie: Word, Outlook, Excel.

Desirable

• Evidence of post-qualification stop smoking work.
• Experience of developing and delivering successful health promotion activities.
• Experience of working in Primary and secondary care settings.

Training

Essential

• Smoking Cessation Training Level 2 delivered by an NHS Stop Smoking Service or the National Centre for Smoking Cessation and Training (or to undertake within 3 months of starting in post).
• Willing to undertake training and development as required to carry out duties of the post.

Desirable

• Specific training on healthy lifestyle activities e.g. healthy eating, physical activity etc.
• Motivational interviewing training.

Qualifications

Essential

• GCSE level English and Mathematics or equivalent,
• NVQ level 3 or equivalent in health related studies OR substantial successful experience of delivering stop smoking interventions in a range of settings

Physical Attributes

Essential

• Ability to undertake the duties and demands of the post. A satisfactory sickness record over the previous 2 years (subject to the need to act with fairness and equality of opportunity, particularly where the sickness is related to a disability and/or pregnancy).

Personal Attributed

Essential

• A current driving licence and access to a car during the working day is essential (reasonable adjustments will be considered for any applicants who are unable to drive due to a disability).
• Ability and willingness to work flexible hours when team / service requires including evenings and weekends.
• Current non-smoker due to nature of post.
• Proven track record of reliability.

Special Knowledge/Skills

Essential

• Demonstrate understanding of human behaviour in relation to making healthy lifestyle changes.
• Good knowledge and understanding of evidence based practice in smoking cessation, the consequences of smoking and the benefits of quitting.
• Ability to build and maintain good working relationships with professionals / individuals from different professional backgrounds.
• Excellent communication (written and verbal) and listening skills.
• Well-developed interpersonal skills.
• Good organisational skills and the ability to prioritise and coordinate own work.
• Enthusiasm and ability to motivate self and others.
• Ability to work on own initiative and as part of a team.
• Understand and maintain confidentiality issues.

Desirable

• Knowledge of existing smoking cessation services.
• Advanced Information technology skills.
• Knowledge of the basic principles of health promotion and issues of health inequality.
• Basic understanding of audit and evaluation.

• Experience of using QuitManager or an electronic clinical record system.
