Barnsleys Older Peoples Community Mental Health Team.

first Band 4 Nursing Associate post.

Main duties of the job
Most importantly, you will be caring and compassionate with
a passion for helping older adults with severe and enduring mental health
conditions.
An awareness of comorbidities is beneficial, as often we
provide care to those with complex needs including psychological, physiological
and social care needs.
Being patient, motivated, adaptable and intuitive will aid
you in providing the best possible care and maintaining our high standards.
You will work collaboratively within our MDT and the wider
health care services, to provide collaborative and holistic care across various
community settings.
Recognising and working within your competence, being an
accountable and responsible practitioner and seeking support as required will
ensure you work safely and effectively.
Identify and positively supporting risk with patients and
their families, Including recognising and reporting safeguarding.
An ability to undertake physical health monitoring including
BP, Venepuncture, ECGs will support our patinets in maintaining wellbeing,
support treatment initiation, titration and yearly wellbeing checks.
Competence in Depot medication administration will support
our patinets in maintaining mental health wellbeing and support our senior
staff in care delivery.
You will be willing to deliver 1;1 and group-based
interventions such as psychosocial interventions, structured clinical
management.
