We are looking to appoint a motivated & forward-thinking
Senior Therapy Assistant to join
the Neighbourhood Rehabilitation Service (NRS) which encompasses the
Intermediate Care team and Long-Term Condition Pathway, working across Barnsley
Community.
The team
provides rehabilitation to patients in their own home & residential care.
The service provides specialist community therapy assessment & treatment to
improve health & independence. In addition, the service offers
rehabilitation to enable earlier discharge from hospital or to prevent hospital
admission.
You will
work as part of a
multi-disciplinary team, following
initial assessment by a registered health care professional you will exercise
initiative & judgement in care planning, goal setting, delivery,
implementation & evaluation of care, working collaboratively & in
partnership with others
You
will contribute to enabling service users to reach their potential & develop knowledge to promote
holistic care, including supporting patients who have longer term
rehabilitation needs.
Hours of work will include weekends and bank
holidays.

Consulting as necessary and working collaboratively within the health care team
to deliver therapeutic interventions, psychological and physical care to a
defined group of service users.
Deliver evidence based planned care,

enables service users to reach an optimum level of functioning, independence, rehabilitation
and recovery.
Provide care within well established
policies, procedures, protocols, guidelines, and established clinical care
plans, whilst acting within the required sphere
of clinical competences for the role

Following
