The Liaison and Diversion Service is
commissioned nationally by NHS England.
It is an all age, all
vulnerability service which offers screening/assessment services to
individuals in police custody and court who may present with vulnerabilities
and who find themselves in
contact with the youth or criminal justice systems as a result of being
suspected of having committed a criminal offence. Vulnerabilities may include but
not limited to mental health, alcohol and substance use, homelessness, learning
disabilities, autism, or other speech and language difficulties.
We provide
court related services which offer greater advice and support to the Judiciary,
including Court Reports and liaising with Prisons where necessary.
L&D
services aim to reduce reoffending, reduce unnecessary use of police and court
time and reduce health inequalities for some of the most vulnerable in society.
It requires a commitment
to close partnership working with other services and working with a complex and
diverse range of individuals with varying needs.
The Associate
Practitioner Role supports the Senior Practitioners within the team and Custody
providing initial screenings and any follow up support that is required with
their caseload, or the Senior Practitioners after an All-Vulnerability
Assessment takes place. The relevant support, referrals or signposting will be
provided, which will require providing support in the community.
Main duties of the job
Following
assessment and identification of any vulnerabilities the practitioner will
either facilitate diversion of the individual away from the criminal justice
system into appropriate services or alternatively with the individuals consent
signpost on to services to meet any vulnerabilities whilst they continue to be
dealt with by the criminal justice system and beyond.
Liaising with
other professionals and service providers to ensure appropriate care and
support is in place is integral to the role.
We are looking for someone who
embraces collaborative working with partner agencies, service users and carers
and who has a keen interest in working with the Police. You will need to be
resilient and have the ability and confidence to work with a high level of
autonomy.
Importantly, you will be dynamic and
persevering in your work, be naturally inquisitive, have excellent risk
assessment and report writing skills, and the ability to quickly establish and
develop rapport with service users.

under direction of the team manager. Providing support with practical concerns
or difficulties that might be affecting service users mental health and
wellbeing.
Provide time limited 1:1 support to
service users, giving information to access a wide range of agencies and
services that can provide help and advice with housing, social isolation,
education, debt management and employment to service users who have been in
contact with the criminal justice system.
Support service users who are typically
hard to engage to access and utilise community services which is likely to
reduce the risk of them reoffending the service users may have a history of
offending, may have co-existing complex needs for example substance misuse and
homelessness and who at times may present with extremely challenging behaviours.
Deliver the highest standards of safe and
compassionate, evidence based care. Thereby enabling service users to reach an
optimum level of functioning, independence, rehabilitation and recovery.
Work collaboratively across a range of
disciplines, developing knowledge and practical skills to perform non-complex,
protocol led, tasks with minimal supervision. To be responsible for own
actions.
Provide care
within well established procedures, protocols, guidelines etc, and established
clinical care plans, whilst acting within the required sphere of clinical competences for the role at all times.
Exercise initiative and judgement in the
assessment, care planning, goal setting, delivery, implementation and
evaluation of psychological/physical care under the supervision of a registered
professional.
