An exciting opportunity has arisen for a Band 6 Dietitian to join our Neighbourhood Rehabilitation Service.
The team provides rehabilitation to patients in their own home & residential care. The service provides specialist community assessment & treatment to improve health & independence. In addition, the service offers rehabilitation to enable earlier discharge from hospital or to prevent hospital admission.
Working as an autonomous practitioner, you will support the delivery of this Intermediate Care Pathway by providing individualised dietetic advice for identified patients in this service, with a predominant focus on nutrition support within the community setting.
You will hold a clinical caseload & work without direct supervision, providing specialist dietetic assessment, diagnosis & treatment programmes to patients. You will also provide training, education & specialist advice to qualified & non qualified health care professionals in all areas of the specialised field.

Main duties of the job
As a NRS Specialist Dietitian you will
Have demonstrable post qualification experience within clinical settings & age groups including older adults, nutrition support & general community dietetics.
Work in partnership with other health professionals to provide holistic patient care.
Work with key members of multidisciplinary team (MDT) to provide, advice, support & care plans to patients.
Have high level communication/interpersonal skills to negotiate & establish collaborative partnerships with patients & care professionals.
Participate in education & staff development, actively participate in regular supervision & appraisals.
Assist in service development & development projects as we strive to transform & modernise patient care.
This is a community-based role applicants must have a driving license & access to a vehicle for work purposes.
A contribution to the delivery of dietetic student training is also expected within this role.
