An exciting opportunity has arisen for a well-motivated, enthusiastic exercise instructor to work for South West Yorkshire Partnership NHS Trust within the cardiac and pulmonary rehabilitation service. This community-based team provides quality care and support for patients undertaking either the cardiac or pulmonary rehabilitation programmes. The post will initially be based at the Rockingham Centre, Barnsley but you will be required to travel within the Barnsley area to deliver the service across multiple locations.

Main duties of the job

The post holder will work as part of a team and will also undertake individual duties. You will be required to manage your own caseload and contribute to the daily management of incoming group tasks. Daily duties include monitoring patients during exercise, planning and progressing exercise programmes, delivering education sessions and recording electronic patient notes.

Experience in delivering the exercise components of pulmonary or cardiac rehabilitation and working within the community setting would be desirable for applicants to this post. It would also be advantageous to have a good understanding of cardiac and pulmonary conditions such as heart attack, cardiac surgery and COPD.

There will be a focus on training and development. The post holder will have regular supervision and will be encouraged to seek training opportunities to develop their CPD.

JOB SUMMARY

To plan, organise, run and evaluate exercise and education sessions for individuals and groups of patients with cardiac or pulmonary conditions within a community setting.

To assess suitability of patients group activity and progress within group where necessary.

To undertake own caseload of individual patients with cardiac or pulmonary conditions.

To manage own caseload of patients by accepting direct referrals from health care professionals. To prioritise, assess, plan, run and evaluate a number of complex activities or treatment programmes for groups or individuals.

Responsible for maintating accurate treatment records and insuring data is entered on to appropriate IT systems for work undertaken.

To develop and deliver specialist therapeutic exercise groups to patients referred into the cardiac / pulmonary rehabilitation service.

To develop and deliver exercise programmes to individuals where necessary, assessing, re-assessing and progressing exercises and discharging patients when treatment complete.

Responsible for organising and delivering educational sessions related to the clinical area.

To lead, teach, advise and demonstrate activities in own clinical area to new or less experienced employees, students or volunteers.

To undertake audits and surveys necessary to work in own area and may occasionally be included in research and development projects.

To ensure own development within the clinical area of cardiac and pulmonary rehabilitation.

To ensure the appropriate use/issue of therapeutic equipment.

To undertake any other duties consistent with the post.
