We are looking for a Senior Healthcare support worker with a positive and progressive outlookwho aspires to provide and promote the best standard of care and support for service users.
The post holder will deliver planned programmes of care and treatment to designated service users, under the direction and supervision of the registered nursing team. Examples include, supporting service users to understand their health needs, promoting annual health checks, completing hospital passports, and providing health action plans. You would also be expected to support service users to health appointments if appropriate, support their understanding in appointments, and provide health education using a range of resources such as easy read. You will be working closely with carers and families, in a range of different care settings, and you will have a case load under the supervision of a named nurse.

Community Learning Disability Health Team in Barnsley.
This is a full-time Band 3 permanent post.
Main duties of the job
You will join an experienced, well-established, and committed specialist multi-disciplinary team who are based in Barnsley. The Team consists of a comprehensive multi-disciplinary team including Learning Disability Nurses, psychiatrists, clinical psychologists, occupational therapists, physiotherapist, speech & language therapists, dietician, therapy assistants and healthcare support workers. The Team works together to support individuals aged 18 and over with diagnosed learning disabilities and their families/carers.
Effective communication
skills are essential and a flexible attitude to delivering services to meet the
needs of service users and their support networks.
To have effective
organisational skills and the ability to prioritise tasks in response to
service or service users need.
To access and engage with
regular management and clinical supervision, raising any concerns in a timely
and efficient manner to a registered nurse or practitioner.
To engage with local
community groups and stakeholders across Barnsley to help improve service
users knowledge and access to health and community services across Barnsley.
Please note, working
hours are 9am-5pm Monday to Friday, excluding bank holidays. Access to a car is
essential.
