An
exciting opportunity has arisen for a well-motivated, enthusiastic Respiratory nurse
to work for South West Yorkshire Partnership NHS Trust within the BREATHE
service.We are looking to appoint to a full time and part time
post.
This community-based team provides specialist
assessment and review of patients with a confirmed respiratory diagnosis.
The
service is responsible for delivering a specialist care to patients in their
own home as well in our community clinics and venues around Barnsley. The
successful candidate will be expected to assess and treat patients with a wide
variety of respiratory conditions.

Main duties of the job
Work as an
expert practitioner, demonstrating evidence-based knowledge, analytical skills,
and specialist assessment to triage, assess and manage a range of complex
presentations.
Work autonomously assessing and managing own clinical caseload supporting
patients with a range of respiratory conditions in a
community setting. This includes those requiring End of Life care and home
oxygen assessment and therapy.
You will be expected to fully integrate yourself within the aligned
neighbourhood playing an integral role within the Respiratory pathway. This
includes participating in MDTs, working alongside primary care colleagues, working
in collaboration with respiratory and medical teams within secondary care and
virtual ward to help facilitate early discharge and care at home, to support
the provision of seamless care.
You will work as part of a team and as a lone
worker and be allocated to a specific neighbourhood; however, travel and work
across all neighbourhoods will be required.

Qualifications

- First level Nurse.
- Educated to Graduate level and/or working towards Masters level.
- Diploma/Degree in Respiratory Disease or working towards.
Desirable
- Professional teaching qualification.
Special Knowledge/Skills
Essential
- Recent up to date knowledge of COPD, Oxygen and Respiratory care including management of acute exacerbations and acutely unwell patients
- Excellent communication and interpersonal skills
- Excellent team working skills.
- Ability to work autonomously and prioritise and organise workload ensuring effective management skills.
- Sound understanding of clinical governance and its implications for services, including experience of quality issues and audit. Ability to apply principles to workplace.
