Job summary
The Clinical Nurse Specialist Team Leader will act as a clinical expert
and will provide professional advice to patients, carers and colleagues for the
Colorectal and Stoma Care Nursing services.
The aim is to deliver high quality patient care, through taking overall
responsibility for the specialist areas of practice and create an environment
to develop excellence within the specialist area of practise in partnership
with other members of the speciality team. You will be required to manage and lead two
specialist areas, these being Colorectal and Stoma Care dealing with both
cancer and benign disease, whilst
actively contributing to the achievements of the Colorectal and Stoma Care
department through effective promotion and management of these services.
The Clinical Nurse Specialist Team Leader will lead and manage as team of specialist nurses, health care assistants, departmental secretary and colorectal clinical coordinator, whilst working in partnership with a community sponsored nurse. The role of the team leader is to ensure that both teams work cohesively to support the needs of the colorectal and stoma patient whilst delivering against service needs in challenging times.
As Team Leader you will ensure that everyone has a job plan and strive to improve care and service delivery. Future planning includes review of the straight to test pathway, delivering the enhanced recovery programme, management of the patient with a high output stoma and capacity planning.
Main duties of the job
The post will coordinate all aspects of audit and research within the
specialists areas of colorectal and stoma care nursing in line with both local
and national criteria and the Peer review process.
The post holder will be expected to manage and lead a team of Specialist
Nurses, Health Care Assistants, Departmental Secretary and the newly appointed
Colorectal Clinic Co-ordinator to ensure and maintain clinical excellence. By providing effective clinical leadership
and coaching of junior members of the team the post holder will enable staff to
become empowered to develop high levels of clinical skills whilst allowing
staff to develop their full potential. Through partnership working you will
lead the development of a highly specialised programme of practice and professional
development for staff within the speciality.The
post holder will provide specialist education and training in a variety of
settings to a range of healthcare staff.
The post holder will contribute to achieving the objectives of the
clinical service group/speciality, corporate objectives and work within the
clinical governance framework and will promote the Trusts Core Values.
The post holder will coordinate all aspects of audit,
undertake research and manage the specialist teams in relation to both local
and national guidelines/measures set for the development of Colorectal and
Stoma Care services. This will include
leading the Annual National Colorectal Peer Review.

- Working as an autonomous practitioner in the field of colorectal and stoma care nursing, responsible for making diagnostic decisions and leading patient care, advising the organisation as a senior nurse with freedom to interpret national and Trust policies within the field of colorectal and stoma care nursing
- Ensure time is allocated to ensure that the post remains within the clinical field, undertaking, developing and working within the service maximising specialist nursing skills, ensuring improved outcomes for patients with Colorectal cancer and benign disease and demonstrate high level of clinical expertise in Colorectal cancer and Stoma Care. Taking into account 4 disease sites of anal, rectal, colon and bladder cancer undergoing ileal conduit formation, mitroffanoff procedure and/or orthoptic bladder (cross over for stoma care aspect liaising with the specialist team) and benign disease. Based of highly developed professional knowledge and vast clinical nursing experience
- To demonstrate high level of clinical expertise based on highly developed professional knowledge and extensive experience in colorectal cancer, colorectal benign disease, urology cancer (crossing over for the stoma care aspect and liaise with the specialist team) and stoma care taking into account 4 areas of disease colon, rectal, anal and bladder
- To apply advanced personal and professional autonomy, whilst acting as an expert practitioner, utilising expert clinical competence and highly developed physical skills e.g. undertaking DREs and abdominal examinations, thus utilising highly specialised knowledge and skills to plan, deliver and evaluate programmes of care which includes undertaking own independent nurse-led 2WW (suspected lower GI cancers) and nurse-led colorectal cancer follow up clinics as per clinic protocol
- To diffuse potentially hostile and antagonistic situations with staff, patients, and relatives, using highly developed negotiation and interpersonal skills
- To request, direct and use
the results of specialist investigations e.g. blood tests, CXRs, USS, CT scans
and MRI scans to assist in the diagnosis and plan patient management/pathways
from a range of interventions.
- Utilise highly developed
specialist knowledge, which is underpinned by theory and experience to
interpret and analyse highly complex clinical and non-clinical facts or
situations, to regularly provide expert advice and second opinion to health
care professionals, including when views may differ or conflict. This includes
the formulation of accurate prognoses and recommendation of best course of
intervention.

assessment efficacy of therapy and amending management plans as appropriate
- To help
provide a specialist service, using complex decision making skills, advising
patients, carers, hospital staff and primary care/community teams.
- To effectively articulate
your expert perspective on a patients condition with colleagues, when various
management options are available.
- To demonstrate an
expert ability to communicate highly complex and very sensitive information to
patients, carers and other staff, where there may be barriers to communication
or may include information about cancer. This must be done whilst ensuring
confidentiality at all times and ensuring sensitivity to the audiences level
of understanding and prior knowledge.

distressing or emotional circumstances through the giving of unwelcome and/or
distressing news to patients, carers or relatives concerning care which can be
either at the beginning of treatment or during the terminal stages of the
disease process, requiring empathy and the ability to reassure patients,
relatives or carers
- Support
patients and their families and ensure that they receive the required
information to enable them to self-manage, lead or participate in their care
delivery. This information can be highly
complex, sensitive or controversial.
- To liaise with Consultant
colleagues within colorectal and oncology services and other members of the MDT
to ensure appropriate patient care management.
- To provide and receive highly complex, sensitive or
contentious information, recognising barriers to understanding this information
and presenting this complex information to large groups of staff in a variety
of settings within the MDT
- Developing
clinical pathways to support management of Colorectal and Stoma Care patients
to prevent re- admission to hospital and providing a community service to those
patients with difficult symptoms/ pathways of care.
- The
receiving and making of direct referrals to and from primary and secondary care
services
- To manage own unpredictable
caseload of patients with complex needs, by planning and implementing highly
specialised, individual complex care packages.
- Perform specialist assessments of patients nursing needs within the
colorectal/stoma care nursing service.
- Plan, implement and evaluate care
delivery using advanced clinical knowledge and expert clinical skills.
- Contribute
towards a multi-professional approach in the management of patients within Colorectal/stoma
care services especially in relation to ongoing care needs and discharge
arrangements

Supervise junior band 7 CNSs within these clinics.
