cancer care, with highly developed leadership skills including the ability to
influence at scale? Do you have experience of working in Haematology outpatient
clinics as a prescriber? We're looking to recruit into the exciting new post of
Consultant Pharmacist Haematology.
The Mid Yorkshire Teaching NHS Trust has a large cancer
service covering oncology and haematology (including bone marrow transplant) and
common cancers across our three hospital sites (Pinderfields, Dewsbury &
District, and Pontefract). You will have the opportunity to share the leadership
on systemic anti-cancer treatments (SACT) as the Trusts Lead Consultant for
SACT, along with our Consultant Pharmacist Oncology.
You'll be working closely with the Lead Consultant for Haematology, Consultant Haematologists, Advanced Clinical Practitioners in Haematology, lead cancer nurses and managers to maximise the safe and effective care of our patients. You will be expected to provide clinical cover to the haematology ward, managing a portfolio of SACT patients, and using your expert clinical practice in outpatient clinics alongside Haematologists, as well as developing the role of pharmacist prescribers in Haematology.

Main duties of the job
You will provide the link with local clinicians and
commissioners, working with the Cancer Alliance, Integrated Care System groups,
and industry to lead regional projects. You will be given dedicated time to
support education, research and development, with opportunities to publish and
undertake practice research.
You will share the
chair of our Systemic anti-cancer Treatments Advisory Group with our Consultant Pharmacist Oncology. Through the
application of medicines optimisation principles, the post holder will help
patients to improve their outcomes with SACT; take their medicines correctly;
reduce wastage of medicines; and improve safety in relation to SACT.
The
post holder will communicate highly complex information about SACT in
appropriate ways to consultant and other clinicians, general practitioners,
pharmacists, nurses, patients and carers in order to achieve the optimum use of
this group of medicines.

order to influence treatment options and ensure compliance with medicines
legislation and avoid errors and side effects.
Participate in joint
rounds with medical consultants and provide highly specialist advice to senior
clinicians on drug treatments for individual patients, applying critical
analysis skills, professional knowledge and judgement where information is
lacking or opinion is divided to influence medical consultants on the
management of pharmaceutically complex cases, outside and beyond any national
guidelines. Be able to conduct an open
discussion when advice is challenged.
Advise medical teams
across MYTT about all aspects of SACT usage, including non-cancer use of SACT
agents and the use of intrathecal chemotherapy where appropriate.
Be accredited in the
pharmaceutical aspects of handling intrathecal chemotherapy and related
products within the current national guidance and legislation.
As a prescriber,
provide a service for patients requiring SACT agents and liaise with other
colleagues where appropriate, including the provision of outpatient clinics as
part of the MDT. This will require very advanced skills and may include complex
caseloads.
Maintaining
appropriate records for independent prescribing. Taking full responsibility for the
prescribing (writing and signing) of prescriptions as necessary.
As an independent
prescriber, provide dose adjustment, advice and prescriptions for SACT
medication with a narrow therapeutic range requiring therapeutic effect
monitoring, including recording relevant biochemistry results in patient
medical records, recording action plans, and adjusting doses to account for
renal or liver dysfunction or other toxicities.
Provide information
and highly specialised advice to patients and their carers to support the use
of SACT agents and other related medication as required.
Provide advice on
pharmaceutical and medicines management to the Medicines Optimisation Group.
