Elderly care services across MYTT is changing. We are looking for up to four dynamic, passionate and highly skilled senior clinicians who demonstrate not only the Trusts values and behaviours but demonstrate significant experience with elderly and frail patients. You will be educated to Masters level in Advanced Practice and will have a wide and extensive range of clinical skills. Our drive to deliver outstanding elderly care both in hospital and within the community will mean that we are searching for experienced candidates to can work autonomously and as part of the wider Consultant and MDT team
Elderly services has an extensive bed base across both Pinderfields and Dewsbury Hospital and as a successful candidate you will be able to continue to expand your experience and deliver your expertise caring for the acutely unwell as well as patients who are medical stable
We are committed to building on the success of the Virtual Ward, supporting patients to stay at home, avoiding admission whilst receiving expert advice, care and treatment
The introduction of Nurse Consultants into the clinical workforce across within elderly is new, and as such as are looking for candidates who have the drive and determination to succeed, bring with them not only experience but ideas and innovations on how we can improve the care we provide

Main duties of the job
The Nurse Consultant will work as an independent consultant practitioner delivering a comprehensive service to patients, employing a high degree of personal and professional autonomy demonstrating highly advanced clinical expertise, competence, levels of judgement, discretion and decision making in all aspects of clinical care clinical care
Demonstrating a sound understanding of the issues related to the identification, assessment, diagnosis, treatment and management of the patient group
Contribute to the clinical and strategic direction for the development of the service, and will be a key member of the senior clinical and management team, providing senior leadership and influencing organisational initiatives related to the specialist area
Lead the development and implementation of a research strategy within the service, resulting in increased research activity and wider understanding of the service
Deliver clinical care for at least 50% of their time with 50% of their time devoted to education/training, research and service evaluation, professional, personal and service development.

The Consultant Nurse will
- Be a role model of expert clinical nursing practice by participating in the provision of care to a definable patient group by using advanced nursing skills expert knowledge, analytical skills and clinical decision-making.
- Spend at least 50% of their time in the clinical setting working with a patient caseload independently assessing patients, implementing and evaluating treatment plans and lead on nurse led discharge from services when appropriate
- Have responsibility for establishing new care pathways in liaison with internal and external care providers to promote a continuum of care (integrated care pathways).
- Lead the co-ordination and delivery of an integrated system of in-patient care, outpatient clinics, virtual ward and complex case management within which referred service users are seen, assessed and courses of treatment are agreed with patients, carers and multi-disciplinary professionals as appropriate
- Maintain strong links with the local Higher Education Institutes with regard to nurse education and will participate in the delivery of education in partnership with all local HEIs as appropriate. These links with higher education will focus on the development of local research agendas which reflect the unique clinical priority areas, thus increasing the relevancy of research activity for local service delivery.
- Act as an ambassador for the organisation and promote its work through presenting and publishing
- Independently prescribe medications/treatments, order investigations and facilitate admission to the appropriate in patient setting or refer on to another speciality
- Work autonomously and accountably, demonstrating expertise, improving the quality of care, health outcomes and experience of patients and families. Undertake other clinical procedures in response to the changes and developments in service provision and patient needs
- Empower others to make safe and appropriate clinical decisions through professional development and expert role modelling Lead MDT meetings and board rounds
- Work in partnership with primary, secondary, tertiary and social care providers to broaden the provision of specialist care and develop joint protocols/care pathways
- Maintain accurate, clear and concise records of patients progress in the medical/nursing case notes or on the electronic patient management systems
