Job summary

The Haematology Care Coordinator role is a fulfilling job role within the clinical haematology service at MYTT.

You will be engaged in face to face and telephone contact with patients who have a haematological condition and will provide them with care and support throughout their journey through their cancer treatment and beyond.

You will work as part of the Haematology Clinical Nurse Specialist team and provide administrative and clinical support to them and the wider Haematology Multi-disciplinary team. You will also be acting as a link between primary and secondary care to ensure a patient has their care holistically treated.

Main duties of the job

Assist in the coordination of referrals to the CNS team and triage to appropriate team member

Triaging incoming telephone calls, managing onward referrals and sign posting to appropriate person or service

To be available to patients, GP practices and Acute service teams

Assisting the CNS team with MDT actions as appropriate

Input information onto relevant computer systems to ensure effective communication between all involved in the patient pathway

Develop and use appropriate communication skills to support contact with patients face to face and on the telephone

Assist where needed in Nurse led clinics

Ensure adequate supplies are available of all patient information leaflets and order more as required

Support CNS team in collection of data to support audits and patient surveys

Understand the patient treatment and information pathways and assist patients in accessing treatment and resources

To be able to complete a holistic assessment and manage immediate concerns, with support from the CNS team

To attend local MDT meetings

To be aware of personal limitations and to seek assistance from other team members as needed

To monitor availability of investigation results and once available to ensure they are made available to the relevant team member

To ensure clear documentation of all interventions made with patient, relative or staff member
