Job summary

This is an opportunity for dedicated, enthusiastic nurses with experience of working with children with a neurodiverse profile who are looking to develop their career within our supportive and progressive Childrens Neurodiversity Community Team, based at Mid Yorkshire Teaching Trust.

You will be a key member of the multi-disciplinary community support team and manage a defined case load of children and their families within the neurodiversity service.

You will be a caring, compassionate individual who is committed to delivering high quality care and ensuring safe practice and a positive patient experience at all times.

Main duties of the job

The post holder will be a valued part of the Community childrens Neurodiversity Team and be expected to work within a multi professional framework, managing a defined case load of service users with Learning Disabilities, ADHD, ASD and other co-morbidities along with complex health needs.

You will require key worker co-ordination of complex cases and ensuring appropriate interventions are in place to support the needs of the service users and their families. This will include completing care planning and risk assessment to manage risks.

Whilst you will be expected to deliver training to families and care provider agencies surrounding challenging behaviour, neurodiverse diagnosis and physical health, you will receive exceptional support and supervision from this established team, working flexibly in varying locations and times to meet the needs of the service.

You will work alongside clients, families, support workers and professionals to enable others to better understand and respond to clients' behaviours that are challenging.

Promote the recognised needs of children and their families diagnosed with ADHD/ASD/LD to improve social inclusion

Lead and promote the ADHD/ASD/LD liaison nurse service and co-ordination in line with the needs of the CYP

Be cost effective in the use of resources

Work autonomously and to be accountable for own professional judgements and actions.

Evaluate and review practise on a regular basis and discuss in Clinical/Safeguarding Supervision when appropriate.
