We currently have additional funding within the Integrated Care Team to support facilitating earlier discharge and preventing hospital admissions. This service provides 24 hour nursing care
throughout the Wakefield Area. The post covers an early, late, long
days and nights shift pattern 7 days per week; including weekend, unsocial and
bank holiday working.
Working as part of the
Integrated Care Team you will be providing care to patients that are managed on
the Integrated Care Team caseload. This includes;

Palliative care at home patients; supporting
patients and families in their own home during their last days to weeks of
life.

Rapid response to patients that require support to
stay in their own homes; preventing hospital admission.

Rehabilitation for patients supporting them in
regaining independence following an injury or illness.

Out of hours district nursing visits.
We are looking to appoint

With a variety of clinical
settings you would gain a wealth of knowledge and experience. We offer a supportive induction period for all new starters this includes shadowing existing team members and training opportunities.
Main duties of the job
To work within the multidisciplinary Integrated
Community Urgent Response Team that delivers community-based services
undertaking nursing and rehabilitation tasks in agreed parameters as set
out by the registered professional.
To carry out care/other duties under indirect
supervision of a Registered Professional.
Act on own initiative in delivering patient care in the Community. Supervision is available as required and work
is within well established procedures and the Trusts clinical supervision
policy.
To support patients to achieve identified goals,
including the practicing of rehabilitation tasks or functional activities such
as self-care, domestic activities.
To assist in maintaining patients skin integrity and recognising
and reporting findings.
To be responsible for the recording of up to
date, accurate information, both written and electronic.
To undertake non-invasive personal cares such as
catheter site cleaning.
To use a range of verbal and non-verbal skills
to establish a professional rapport with patients and their carers.
Participate in being cost effective in the
delivery of high quality care.
Travel across the district of Wakefield is part of the role, and travel expenses associated with patient visits will be paid via expenses claimed.
