Experience is beneficial with knowledge of community services, and an interest in discharge and patient flow.

We are looking for an individual who has the confidence to challenge decisions at all levels, experience of working in a fast paced and high-pressure environment, and the ability to recognise the need to escalate complex discharges to senior management to prevent discharge complexities.

We have a reputation for providing a caring and supportive style working environment for our staff and you will find that we are determined to work towards improving health, preventing unnecessary admissions to hospital, and facilitating early discharges by caring for patients.

Main duties of the job

This role will work as part of the Early discharge assessment team, through a multi-disciplinary team approach working alongside colleagues from Therapy services, Social services and the Acute trust. Promoting positive staff behaviour, and facilitating the delivery of treatment to individuals, groups, and communities.

To work collaboratively to support the delivery and development of integrated care across the Wakefield District, working within the acute trust.

The successful applicant will have good communication skills, IT skills, and a flexible attitude to working patterns and environments. Your work pattern will include shifts and weekends. You should also have a full clean driving license, and access to a vehicle for work purposes.

Experience of working in the community is essential.

This is an exciting time to be part of the Integration agenda in Wakefield.

If you like our vision and values and feel that these are you, we encourage you to apply as at Mid Yorkshire Teaching NHS Trust, we like to work with like-minded people!
