An exciting development opportunity has arisen
for a dynamic and motivated Registered Nurse to join our Integrated Care Team
on a permanent basis as a Band 6 Nurse Coordinator.
We currently have a vacancy
within the Integrated Care Team; a service that provides 24 hour nursing care
throughout the Wakefield Area. The team cover early/late/night shift pattern 7
days per week; including weekend, unsocial and bank holiday working.
We are looking for an enthusiastic and forward-thinking
individual with excellent leadership skills to facilitate and participate in
this developing team.

Main duties of the job
Working as part of the Integrated Care Team you will be providing care to patients that are managed on the Integrated Care Team caseload. This includes;
- Palliative care at home patients; supporting patients and families in their own home during their last days to weeks of life.
- Support with Urgent Community Response 2 hour or same day visits.
- Rehabilitation patients; supported them in regaining independence following an injury or illness.
- IV/AB @home - Intravenous Antibiotics at home team.
The post holder will be responsible for coordinating shifts and ensuring the safe delivery of patient care within patients own homes and maintaining high standards of care.
The successful applicant will have good communication skills, IT skills, and a flexible attitude to working patterns and environments. Your work pattern will include shifts and weekends. You should also have a full clean driving license, and access to a vehicle for work purposes.
Experience of working in the community is advantageous but not essential as a full induction program will be offered.

To provide Intermediate Care Nursing including End of Life Care, Palliative Care, and IV Antibiotic Therapy in the patients home with the aim of reducing hospital admissions facilitating timely discharges, reducing in-patient bed days, and maximising patients independence allowing them to remain in their own homes wherever possible.
The post holder is responsible for the management of the Integrated Care Service caseload, and the Co-ordination of patient care. Working as an autonomous practitioner to provide a high standard of clinical expertise in the assessment, planning, treatment, education, and advice given to patients, carers, and other health and social care professionals. To have ongoing responsibilities for the implementation, evaluation, and re-assessment of patient needs.
This role supports the Team Leader by supervising and leading the staff within the Integrated Care Team in their absence. Promoting positive staff behaviour, and facilitating the delivery of treatment to individuals, groups, and communities.
To work collaboratively to support the delivery and development of integrated care across the Wakefield District, in a community based team, including End of Life Care, Palliative Care, and IV Therapy, ensuring a seamless facilitation of care and treatment within a 24 hour, 7 day service.
Key Responsibilities
To supervise and lead staff within the Integrated Care Team in the absence of the Team Leader promoting positive staff behaviour and facilitating the delivery of treatment to individuals, groups and communities.
To take full responsibility of the service caseload in the absence of the Team Leader.
To analyse and triage referrals received by the service against the eligibility criteria and using own clinical expertise. Taking appropriate action to ensure the health needs of patients are met.
To provisionally identify with the referrer what the goals and aims of the interventions will be.
To adopt a problem solving approach for referrals considered not appropriate for the service, sign posting and redirecting to other services as appropriate.
To discuss contentious referrals with the Team Leader or Locality Matron in their absence.
To act in a professional manner at all times whilst on Mid Yorks business and demonstrate excellence in clinical practice.
To demonstrate and deliver high levels of holistic assessment within the patients own environment, utilising appropriate assessment tools, contribute to the development of patient centred care plans and to make referrals as appropriate to other agencies. To understand and have awareness of the importance of collaborative partnerships with other multi-disciplinary agencies. To assess and plan individual patient care plans, education and treatment reviews.
To effectively discharge patients from the service and ensure patients are sign posted and/or referred to other services either statutory or voluntary should ongoing health or social needs be identified during discharge planning.
To perform risk assessments ensuring a safe environment within patients homes and work areas. Ensures adherence to organisational Health and Safety, risk and governance arrangements.
To demonstrate skills in communicating with patients and relatives/carers who have barriers to understanding such as language, sensory impairment, confusion and aggression ensuring ethnic considerations take place.
To actively be involved in the promotion of healthy life styles, secondary prevention, health promotion and patient education.
To work autonomously to competently administer intravenous therapy, cannulation and phlebotomy. To maintain own knowledge base and experience that reflects the current evidence based practice in relation to IV Therapy.
To lead in the continued development and provision of a Community Integrated Care Service which will impact to reduce hospital admissions, facilitate discharges to reduce in-patient hospital bed days and reduce the incidences of healthcare associated infection.

Review medications, seeking advice from local pharmacists and the drugs information hotline as appropriate. If legally authorised, prescribe medications from agreed formularies based on efficacy, safety and cost.
