Job summary

An opportunity has arisen within the MY Therapy service to recruit a highly motivated and experienced Band 6 Occupational Therapist on a part time (15 hours per week) basis.

MY Therapy service is a large and very busy MDT incorporating Occupational Therapists, Physiotherapists and support staff. We are proud to have played a large part in the Connecting Care Closer to Home agenda for Wakefield. We are based across the Wakefield District covering several bases. Our team is flexible and the successful candidate may be asked to work across a variety of bases.

NHS and Council workers, together with the voluntary sector in Wakefield District, have been part of this scheme to pioneer integrated care between a number of organisations in particular Health, Social Care and the voluntary sector to deliver more joined up services closer to home where possible for citizens. The aim is to help people to remain more independent for longer whilst receiving health and social care in their own home where possible.

The Community was rated as Good when last inspected by the CQC and the successful candidate will instil the Trusts values and behaviours in ensuring quality services continue to be delivered and to strive for further improvement.

Main duties of the job

The successful candidate will work together with colleagues, partners, patients and their families/carers in providing rehabilitation in the community thereby preventing unnecessary admissions to hospital / residential settings whilst also facilitating discharge. We are an agile workforce and deliver care to patientsacrossthe Wakefield district.

MY Therapy also provide therapy to the Wakefield Intermediate Care unit providing rehabilitation to patients prior to discharge into the community. We also provide an urgent community response service and specialist neuro, cardiac and respiratory and long covid services to patients in the community. We run an internal rotation in the team with opportunities to gain experience in some of these specialist areas.

Senior staff are expected to take an active part in the leadership of the team and offer support to junior and support staff and students. We encourage all staff to be instrumental in service development and alsooffer: -

To be responsible for the assessment, prescription and evaluation of specialist equipment and adaptations for patients with multiple and complex needs.

To demonstrate a detailed clinical knowledge of a wide range of medical, respiratory, neurological, musculoskeletal, and orthopaedic conditions.

To be able to use research, clinical reasoning and problem solving skills to identify the therapeutic benefit of client centered interventions and make professional judgments with individuals with diverse or complex needs, or multiple problems according to outcome, changes in patient needs and environmental influences.
