Job summary
We have an exciting opportunity for an enthusiastic and patient focused pharmacist to join our team to lead on Palliative Care and pain services.
You will provide a clinical pharmacy service to our local hospices in Wakefield and Pontefract, as well as working with the palliative care and pain teams within the acute Trust.
This part time (25 hours) role will suit an experienced pharmacist with a strong clinical background and excellent communication skills. You will be adaptable and confident working within a multi-disciplinary team.
Main duties of the job
To provide a clinical pharmacy service to our local hospices in Wakefield and Pontefract. This includes clinical duties such as medicines reconciliation, patient counselling and clinical review of medicines prescribed.
Work with the MDT to provide patient centred pharmaceutical care.
Provide education and training to clinical staff regarding medicines use in end of life and pain management.
Lead the development, review and implementation of trust wide evidence based policies and guidance relating to the specialist area.
