Job summary

Mid Yorkshire NHS Teaching Trust has been awarded new funding to implement and provide a dietetic-led Off-FP10 service for enteral (HEF) and oral nutritional support (ONS) across Wakefield Place. Working in partnership with Wakefield Health and Care Partnership and the ICS, this service will ensure all patients requiring nutritional support are referred to and reviewed/monitored by the dietetic team. This new development will provide a patient-centred, needs based service, which will allow seamless and more timely care for community patients who may require nutritional support intervention. The aim of the new service is to stop inappropriate prescribing of ONS (in line with NHS England intentions), ensure all patients receive timely and appropriate dietetic intervention and provided regular and ongoing training to all care homes across Wakefield Place. The service also aims to reduce health inequalities by ensuring improved access to dietetic support for all patients, regardless of location. The service has been commissioned on a permanent basis, so we are looking to recruit one permanent and one fixed-term dietitian.

Main duties of the job

To provide cover for colleagues during spells of absence (e.g. annual leave), as determined by service needs. To give talks and presentations, e.g. wound care, cardiac rehabilitation, care home training. Provide health promotion advice on study days etc. to health professionals and members of the general public. To be professionally and legally accountable and responsible for all aspects of own work, including the management of patients in your care. To develop and implement treatment plans for patients following assessment and analysis of diagnostic criteria, nutritional history, biochemical and clinical data, and social/psychological circumstances. To be an integral member of the team in setting up, maintaining and evaluating a fully operational Off FP-10 model in the Wakefield community working closely with colleagues of all bandings. To help support the patients in solving issues and admin related problems in relation to prescriptions of nutritional supplementation. To continually develop and maintain practical and theoretical skills through continuing professional development by attending relevant study days, tutorials, and through case load experience. To produce evidence-based information in various media for all users, To know the professional and personal scope of own practice and make appropriate and timely referrals to other agencies.
