The Recovery Support Worker role within the Rotherham Assertive Outreach and Community Rehabilitation Team involves providing practical, recovery-focused support to adults with complex mental health needs in community and inpatient settings. Working under the guidance of a multidisciplinary team, the post holder supports care delivery through a care programme/case management approach, contributing to individual and group-based interventions that promote social inclusion, independence, and mental health recovery. The role requires strong communication skills, a person-centred approach, and a commitment to ongoing training and professional development.
Main duties of the job
Deliver mental health care and support within the community setting according to the agreed care plan, working independently where indicated by a risk assessment.
- Support patients in accessing local community resources.
- Undertake and adhere to basic risk assessments.
- Provide interventions as specified in the care plan, working independently when indicated by a risk assessment.
- Provide health education.
- Offer support to patients' families and/or carers.
- Complete patient-reported outcome measures (PROMS) in accordance with trust guidelines.
- Liaise with other health, care, and support agencies as appropriate.
- Establish and maintain effective communication with colleagues, service users, carers, and other professionals.
- Provide information and advice to patients and carers.
- Attend and contribute to MDT meetings.
- Maintain professional confidentiality.
- Demonstrate an awareness of clinical governance and risk management.
- Maintain accurate records of clinical contact, mileage, and activity reporting daily, in accordance with Trust and Local policies and procedures.
- Maintain comprehensive and accurate patient records.
- Complete RADAR incident forms as required.
- Actively participate in supervision and reflective practice.
- Participate in an annual Personal Development Review.
- Attend all mandatory training and additional training relevant to the role.
