Job summary
The post holder will be based within North Lincolnshire supporting the operational oversight of Liaison Services. This consists of two different teams: Hospital Liaison and Care Home Liaison.
Hospital Liaison Mental Health Services are specialist services providing mental health care in a physical health setting. This role is providing a liaison mental health service to Northern Lincolnshire and Goole NHS Foundation Trust, based at Scunthorpe General Hospital and operates as part of a Core24 Model, providing 24/7 services.
Care Home Liaison Services offer direct access for care homes to specialist mental health services for appropriate and timely assessment of the needs of a person living in a care home. The team covers the geographical area of North Lincolnshire and work in collaboration with other multidisciplinary team members to provide a safe and effective service.
Main duties of the job
- To provide support to the service manager to meet the aims and objectives of the service including deputising and delegated responsibilities.
- To provide support to the service manager in the effective day to day management of the team within the resources available and agreed governance frameworks
- To have operational oversight in ensuring the Team meet the Evidence-Based Treatment Pathway (EBTP) Clock recommended response times for mental health crisis as informed by NICE Guidance.
- Person-Centred/Coordination of Service users care:
- Ensure the safety and quality of the service user's journey by effective planning and co-ordination of the episode of care, including the smooth transition to other settings.
- Ensure effective admission or referral/discharge planning with relevant multi-disciplinary and multi-agency teams as required or ensure robust referrals / access to services are in place with a regular review of caseloads within the team(s
