Job summary

The Care Home Liaison team (CHLT) offers assessments and interventions to support patients dealing with various mental health issues, including dementia, residing in care homes.

This service is accessible to patients over 65 living in residential care or nursing homes, and those under 65 diagnosed with dementia.

The Band 7 Clinical Nurse Specialist will provide advanced clinical expertise, leadership, and consultancy to care homes within the North Lincolnshire geographical area. The CNS will work in partnership with care home staff, GP's, primary care networks, community services, mental health teams, and acute trusts to support high-quality, person-centred care for older people.

The post holder will play a key role in reducing unnecessary hospital admissions, supporting timely discharges, and improving outcomes for care home residents through education, clinical support, and service development.

Main duties of the job

  • Deliver specialist, evidence-based assessment and management of individuals under the services care with complex health needs, including frailty, dementia, and long-term conditions in accordance with NICE guidance.
  • Provide advanced clinical advice and support to care home staff to enhance their knowledge and skills in managing patient's health and wellbeing
  • Lead on developing, implementing, and evaluating individualised care plans in collaboration with multidisciplinary teams
  • Act as a clinical leader and role model, promoting high standards of care and best practice across care homes.
  • Provide specialist input to case discussions, safeguarding concerns, and multidisciplinary reviews
  • Support care home staff with complex decision-making and ethical issues, including capacity assessments and advance care planning.
  • To determine management plans, and where appropriate, deliver short term psychological treatments, for people who are referred with complex mental health needs based upon a comprehensive psychosocial, risk and needs assessment of that individual.
  • To represent the Trust from a Mental Health Perspective and attend relevant meetings for your role to promote collaborative working and build on working relationship with internal and external stakeholders.
