Job summary
The post holder will be part of the Home First Service Delivery this will require to work across and develop skills in all areas within the pathway, these include:
Rapid Response service
Unplanned Physiotherapy
Community Rehabilitation Team (Formally Reablement)
Specialist Falls service
Hospice
Virtual Ward
Intermediate Care Ward
Enhanced Care Home Team
To contribute to the delivery of a 7-day working service. Although the role will be predominantly be within the Enhanced Care Home Team and Community Sector, there may be an expectation to complete assessments, support team or work on inpatient rehabilitation wards as required.
Main duties of the job
o To provide specialist physiotherapy assessments over a range of conditions and complexities using advanced clinical reasoning to analyse clinical and non-clinical findings to form accurate physiotherapy diagnoses and prognoses.
o To assess patient understanding of assessment and treatment in accordance with the Consent Policy, and to assist members of the team to work within the legal framework for patients who lack capacity to consent to treatment.
o To provide health education, health promotion and prevention advice.
o To manage a clinical caseload, organising this effectively and efficiently with regard to clinical priorities and use of time with access to expert support as required, both for self and more junior staff, within MDT.
o To formulate and recommend to others the best course of physiotherapy intervention and/or treatment programmes to meet the agreed goals set with the individual patient/or carer, using evidence based practice and a wide range of treatment skills and options.
o To evaluate patient progress, review goal setting and alter treatment programmes as required.
o To use a range of verbal and non-verbal communication methods to communicate effectively with patients and carers to maximise rehabilitation potential.
