Job summary
As a team, we are proud to be in a position to offer a secondment opportunity for a highly motivated and innovative practitioner into our successful Young People's Health And Wellbeing Service.
You will be expected to deliver early help and brief interventions, public health messages and clinical care to Children, Young People and Families within a variety of locations such as schools, homes and other appropriate settings including virtual platforms, under the support and guidance of senior members of the team.This will include delivering education and supporting Young People around Child Exploitation, Substance Misuse, Vaping and Sexual Health.
You will be involved in delivering outreach work, to ensure that the service is visible within our communities and that we are meeting the needs of the Young People across Doncaster.

Main duties of the job
- To contribute to promoting health and well-being and support families, children and young people to make positive choices about their health.
- Work within identifiedpublic health priorities to design, plan and deliver activities and services to reduce health inequalities across Doncaster
- Be involved in partnership liaison, collaboration and networking through outreach work in order to promote referrals to the service.
- Undertake initial screening or assessments using a range of tools e.g. and substance misuse screening tool, outcome stars assessments, CSE checklist, spotting the signs and review the assessments within 1:1 clinical supervision
- Contribute to the formulation and development of individual care/recovery plans and implement the agreed aspects of the plan in conjunction with other staff.
- You will have experience of working with children, young people and their families in either a health, or community setting so that you are able to transfer your skills and have a full understanding of the demands of this role.
