Job summary

Are you a experienced Social Worker looking for a new challenge or looking to take the next step in their career? If so then Doncaster Community Team for Adult mental health would love to hear from you!

The successful candidate will work with adults with symptoms of mental health within the area, and will need to be able to work both independently and as part of a wider multidisciplinary team and other partners.

The post will involve working in a variety of settings including patients own homes, care homes, supported living environments etc.

We are a growing team with close links to our colleagues in both Rotherham, Doncaster and Scunthorpe. We will be able to provide regular support and supervision. Our priority is to ensure that we provide the best patient experience and clinical outcomes possible. We offer a commitment to CPD and value your skills and ideas. Main duties of the job

The main duties of the role are;

oTo be responsible for a caseload, undertaking holistic, strengths-based assessments with adults, carers, and families to help them make informed choices and decisions, enabling them to clarify and express their needs, manage risk and contribute to service planning.

oProvide written/verbal records and reports which are concise, factual, and informative based on analysis of evidence which can be used within the legal context and where necessary give evidence in court in relation to care or other proceedings. (With support, guidance, joint work where necessary on complex cases)

oUsing evidence-based practice and a strengths-based conversations, identify need and take appropriate action, which is outcome focused.

oPromote independence and reduce risks for Adults with Social Care needs living in Doncaster.

oTo assess and manage risks appropriately.

oTo participate on the team duty rota

oProvide peer support to colleagues; advice and guidance to Assessment Officers, new employees and student Social Workers
