Job summary
To work within the Mental Health Urgent Care Service's Home Treatment Team (HTT) to undertake all aspects of clinical duties as a clinical lead within the team. To work with patients with particularly complex needs. The aim of HTT is to provide patient-centred intensive care and treatment to patients and their families at home as an alternative to hospital admission.
Crisis care is a highly specialist intervention and involves supporting our most vulnerable patients and their families in the least restrictive setting possible, by providing a variety of recovery focused psycho-social interventions that enable the person to regain or maintain independence.
Main duties of the job
You will ensure that the care delivered is excellent and in keeping with the most up to date guidelines and best practice.
To work with carers/family members as necessary to provide a thorough bio psychosocial assessment of their needs. Carers and significant others will be involved where appropriate in collection of risk information and in the development of a management plan.
To take an advisory and supervisory role in partnership with the clinical team in managing complex cases.
To lead and present complex, sensitive, and challenging clinical and professional communication processes that may impact on service users, carers, staff, the team, the organisation, partner agencies and members of the public, where there are barriers to understanding.
Safeguarding the health and wellbeing of children and vulnerable adults, by being familiar with and adhering to Trust safeguarding policies and participatingin relevanttraining.
To utilise problem solving and decision-making skills to support safe discharge planning.
The post holder will regularly deal with difficult and distressed patients and carers, conveying complex, difficult and unwelcome information relating to both staff and clinical issues. This may include occasional exposure to verbal and physical aggression.
You will provide a modelling role to all staff; ensuring standards of care are met.
