INTERNAL VACANCY FOR ROTHERHAM ASSERTIVE OUTREACH STAFF - 10 MONTH SECONDMENT OPPORTUNITY - JAN 2025 - OCTOBER 2025.
Rotherham, Doncaster and South Humber (RDaSH) NHS Foundation Trust are looking for a confident and enthusiastic individual who is passionate about co-production and working in mental health rehabilitation. The postholder must have previous experience of working with service users who have severe and or enduring mental health or addiction issues.
The postholder will be based in the Rotherham Assertive Outreach Team, supporting the multidisciplinary team to embed the role as the service continues to develop and supporting patients in their own home and community settings. We are seeking someone who is able to support and grow the pathway, as the role develops through transformation.
Candidates with experience in direct clinical support and leading structured groups, are welcomed.
Main duties of the job
To support in the assessment, delivery and evaluation of individualised holistic therapy plans and treatment goals, devised in collaboration with the service user and the wider MDT. This will involve working within both 1:1 and in group settings, delivering sessions focused on rehabilitation and recovery for service users that may have complex mental health needs.
The post holder must be confident working across traditional service boundaries without direct supervision, using technical, creative or teaching skills, to provide and co-produce therapeutic activities in the rehabilitation pathway, community settings and the service users' own homes as delegated by senior staff. This is a wonderfully varied role that will enable the right candidate to develop their skills as a Band 4 practitioner.
The aim of therapeutic activity is to support service users who are occupationally deprived to develop their skills of daily living, make community connections, engage in social, leisure, wellbeing, and vocational aspirations, and develop meaningful routines, in order to reduce admissions to hospital, reduce symptoms and improve service users' quality of life.
You will assist the Wider MDT, in developing links and partnership working with a variety of Voluntary, Community and Social Enterprise agencies, and contribute to the ongoing assessment, planning, delivery and review of activities and interventions, against identified health, social care, wellbeing and recovery needs.
