Job summary

The Adult Community Mental Health Teams within Rotherham, Doncaster and South Humber NHS Foundation Trust has an exciting opportunity for a Band 6, Care Co-ordinator to join the East AMH Locality Team. As part of the Transformation of Mental Health Services, Doncaster has exciting changes ahead and is looking to appoint additional care-coordinators to support the new services.

The East Locality Team provide support to individuals with complex and enduring mental health issues. The team offers a range of management strategies and interventions including various psychological therapies. Care and support is offered in line with the CPA approach.

Applicants must demonstrate a flexible approach, evolving around the needs of service users, their families and significant others. The role involves working as a team member, with the responsibility of a caseload across traditional boundaries of Adult and the voluntary sector services. The service ethos is focused on recovery and social inclusion and therefore offers successful applicants an exciting and demanding opportunity to improve people's lives.

We are looking for an enthusiastic person with excellent team working skills, who is keen to develop their knowledge and skills in this area. You should be a competent and motivated practitioner with appropriate demonstrable experience. You will work as part of a multi-disciplinary team providing assessment and therapeutic intervention in the community

Main duties of the job

To manage an assigned caseload delivering continuous effective evidence based care.

To carry out comprehensive patient centred assessments, plan and implement individual strategies of care to meet identified needs in conjunction with the patient, care giver and members of the multi-disciplinary team as appropriate.

To evaluate care plans and outcomes regularly and modify as required.

To fulfil a care co-ordinator role and ensure that all care given is in line with the requirements of the Care Programme Approach/ care management using SystemOne as the primary patient care record.

To attend reviews, case conferences, hospital ward rounds, tribunals and other meetings as required.

To liaise effectively with all members of the multi-disciplinary team and other agencies involved in the care of the individual thereby maintaining and enhancing close professional working relationships.
