Job summary

As a member of the Primary Care Network Mental Health Team the post holder will provide high quality nursing interventions, within the Routine, Dementia Review and Severe Mental Illness (SMI) Health Check pathways, as well as other specialist Mental Health Services.

Be responsible for the promotion and assistance in the maintenance of the highest attainable standards of nursing care provided by the team.

Provide practical support and advice to the team members ensuring they are visible to all staff to provide clinical leadership across the Primary Care Network Mental Health Team.

Main duties of the job

To lead and model a robust triage process for all referrals in to the PCN MHT

Interface with other agencies both within the Trust and wider stakeholders

Develop clinical pathways through involvement with the clinical team

Managing demand on pathways and waiting lists, escalating concerns to the Team Manager

Having oversight of referrals into the team and ensuring appropriateness for each clinical pathway

To assist managers and the Trust to develop and improve services and practice for patients and staff

To provide clinical leadership, guidance, support and coaching to staff, promoting effective change management, and strengthening of other staffing leadership skills

To plan, organise, undertake, and document full bio-psychosocial assessment for patients referred to service

Work closely with medical colleagues to determine appropriate treatment plans

To communicate diagnosis and treatment options to the patient and family

Participate in appropriate training as required by the Trust
