Job summary

To provide support, give time to older people with complex mental health needs in order to promote their recovery in the community.

To work directly with older people with complex mental health needs on an individual and/or group basis, wherever they may be within older people's mental health services- working across traditional service boundaries.

To assist the care coordinator to provide care through the care programme/care management approach.

To develop CEW intervention plans with agreement from the service user and care coordinator.

You will be required to work on your own for periods of time without direct supervision.

Main duties of the job

Provide practical and regular support to service users and carers in community or inpatient settings, with a focus on social inclusive activities and recovery, e.g. escorting service users when accessing local resources, assisting to complete benefits forms, attending a recreational facility.

Participate in group activities within Trust premises and community settings. Records and reports back fully on activities undertaken and alert others in the care team to any issues that arise during the activities.

Attends/participates in meetings/case conferences/single point of access meetings as required in relation to allocated service users.

Complete written documentation; provide regular written and verbal reports about the service user to care coordinators and other professionals involved in the care package.

Act as an advocate within appropriate and agreed boundaries of confidentiality/professionalism. Use supportive, active listening, non-verbal skills and body language and questioning skills.

The post holder must have the individual service user's needs at the fore at all times and use the agreed set of values and skills for CEW workers to underpin their day to day work.

Provide regular and practical support to service users and their carers/significant other in developing and managing dignity and independence.
