Primary Care All Age Mental Health Practitioner

Rotherham Doncaster and South Humber NHSFT

The closing date is 22 October 2024

Job summary

RDaSH, in partnership with Primary Care Networks and Voluntary Community and Social Enterprises, have an exciting opportunity for enthusiastic, motivated and caring First Contact Mental Health Practitioners from a Mental Health Nursing, social work or Occupational Health background to work within the expanding Primary Care Networks (PCN) across RDASH and local communities. We know that the first contact with services for emotional and mental health support is the most important part of the journey to recovery. We see this role as fundamental to the development of innovative and integrated mental health services within our local community. The role will help us achieve our shared ambition of 'no wrong door' and the delivery of the right care, in the right place, at the right time, and with the right service.

We are looking for Mental Health Practitioners with exceptional knowledge and demonstrable experience in mental health management to provide high quality care at a local level. You will be aligned to the PCN to support the community mental health transformation programme and will work autonomously screening and triaging and will undertake mental health assessments and arrange follow up in Primary Care as appropriate, to provide therapeutic interventions. You will work in the context of local and national drivers providing clinical care for identification of individual needs which essentially supports diagnosis and treatment options.

Main duties of the job

This is an exciting and innovative post to support the needs of the local adult and aging population living with a wide range of mental health difficulties. The role holder will fulfil a first contact specialist mental health role across the Primary Care Networks (PCNs). Within the primary care setting, the role holder will work in a collaborative and multi-agency way.

The post will facilitate the interface between Primary Care and Secondary Mental Health Services to provide specialist advice and support, assessment, treatment, education and solution-focused approaches to the local Primary Care Teams, for patients and carers.

The role will offer expertise in networking and navigation to promote ease of access to a range of specialist mental health support services. This ensures that each person, once assessed, receives the best possible care in the right place at the right time and by the right supporting service.

The post holder will be an effective member of the multi-disciplinary team. They will be professionally accountable and responsible for patient care, undertaking initial assessment with signposting to appropriate services, or providing time-limited psycho-social therapeutic interventions directly, in e.g. GP surgeries and other primary care or community settings.
