Job summary
1.1 To promote and maintain the highest attainable standard of nursing care in a community setting. 1.2 To screen referrals for the team as allocated by the Team Leader through the Single Point of Access Protocol and cover duty days.1.3 Perform initial Mental Health Assessments 1.4 To maintain close working relationships with referring agencies. 1.5 To deputise for the Team Leader when required. 1.6 To carry a caseload of patients with both functional and organic issues as identified through the assessment process.1.7 Manage and display a positive attitude towards changes as set out within service requirement literature i.e. 5 Year Forward Doncaster Place Plan 1.8 Although variations are expected, this post will have a notional split of: Caseload Management 80%

Education, Research & Development 10%
Main duties of the job
2.1 To work as a member of the integrated Multi-Disciplinary Team reporting back to colleagues, both verbally and in writing, seeking guidance where necessary. Facilitation of multi-agency working. Use of electronic patient record (SYSTMONE) as per Trust and CPA Policy.
2.2 To provide each patient for whom they are the named nurse with an agreed care plan, containing review dates, in accordance with Community Mental Health Team operational policy and Care Programme Approach standards. Post holder will actively seek to contribute to setting standards of care and other quality initiatives.
2.3 To promote compliance with medication and other treatments prescribed, noting and reporting non-compliance and side effects as appropriate. Administer depot injections adhering to the Trust policy.
2.4 Liaise with General Practitioners and other agencies. Fulfil the requirements of the Care Co-ordinator role within the context of the Care Programme Approach. 2.5 To advise patients, carers and significant others of the available resources. Signposting patients to agencies best able to meet these in the community and facilitate smooth transition to such resources.
2.6 To understand the requirements of the Mental Health Act (1983) and its application in a community setting.
