Job summary

Job Role Summary

  • The role entails providing a mental health social work service to those in need of care and support within a multi-disciplinary setting, in partnership with a range of other professionals.
  • The post holder will be responsible for undertaking person-centred assessments, outcome-based care planning and reviews in line with the Care Act 2015.
  • The post holder will undertake direct work to people in need of care and support services and carers and apply their knowledge and skills in respect of Safeguarding, the Mental Health Act 1983 (amended 2007), Mental Capacity Act 2005 and Human Rights Act 1998 and associated case law.

Main duties of the job

The Post holder will work with those who require care and support services in a range of settings, working in partnership with them to agree what outcomes they hope to achieve. This will be through assessment of need using strength based and human rights approaches with an emphasis on prevention and independence.

The post holder will work to both the Trust and local authorities' operating model to arrange any packages of care which may be required and will work closely with colleagues in other Trust services such as community mental health and in-patient care as well as other partners and providers of care. They will provide a social care perspective to the Multi-Disciplinary Team setting within adult mental health.
