Job summary
The East Locality Community Mental Health Team within Rotherham, Doncaster and South Humber NHS Foundation Trust has an exciting opportunity for a Band 5, Community Psychiatric Nurse to join their team and to help make a difference to people in the local area.
The East Locality Team provides support to individuals with complex and enduring mental health issues. The team offers a range of treatment interventions including medication monitoring and administration, and various psychological therapies. Care and support is offered in line with the CPA approach currently.
Applicants must demonstrate a flexible approach, evolving around the needs of service users, their families and significant others. The role involves working as a team member, with the responsibility of a caseload across traditional boundaries of adult and the voluntary sector services. The service ethos is focused on treatment, recovery and social inclusion and therefore offers successful applicants an exciting and demanding opportunity to improve people's lives.
Main duties of the job
- Managing a team caseload under supervision in accordance with sound nursing practice and principles.
- Undertaking regular supervision and PDR in accordance with Trust policy.
- Promoting and Maintaining the highest standards and attitudes towards the care of patients / clients. Assisting colleagues in maintaining an effective and efficient service.
- Assisting, developing, implementing and evaluating plans of care, ensuring therapeutic and recreational programmes for patients / clients individually and in groups, in the Community.
- Ordering, storage, monitoring and administration of medication.
- Liaison within the multi-disciplinary care team, including carers, voluntary agencies etc.
