Memory Service within the Community Mental Health Team (CMHT) for Older People

Clinical Dementia Advisor (CDA), Band 5 (this post is open to all Registered Healthcare Professionals)

Location: Central Locality, Tickhill Road Site, Balby, Doncaster

Full time 37.5 Hours, permanent post

This is an exciting opportunity to join a new service model of post diagnosis support for people affected by Dementia. RDaSH is the lead provider in partnership with the Alzheimer's Society. The clinical staff will be employed by RDaSH and work jointly with the non-clinical staff from Alzheimer's Society.

By joining in the development stages of this new service model the post holder will have a unique opportunity to be involved in the planned events to promote and grow the provision to ensure it meets the needs of people using it. The CDA will be instrumental in establishing the services role in the RDaSH footprint and the wider health and social care community, they will be liaising with colleagues in Primary Care and third partner agencies.

The CDA will be Locality based in the Central area in line with the Doncaster Place Plan. They will be an integrated part of the current CMHT provision and will have the full support and receive supervision from the Multi-Disciplinary Team that is already established.

Main duties of the job

The post holder will work as a member of a multidisciplinary CMHT, the post will be for those patients that have already been assessed and given a formal diagnosis of dementia by the team.

The model for the post diagnostic care is a tiered step-up and step-down model of care in partnership with the Alzheimer's Society led by RDaSH. This post is for the moderate level of patient need that requires clinical advice and interventions with multiple complexity of need to prevent break down in the community.

To be the lead professional for the review and coordination of care needs, planning and implementation of evidence-based care to this defined patient group and their carers as part of a multidisciplinary approach.

To provide professional advice and support to other agencies and members of the multidisciplinary team. To act as a role model and resource for junior/less experienced staff.
