Job summary
Rotherham, Doncaster and South Humber NHS Foundation Trust has an exciting opportunity for a Band 6 (22.5 hours per week) Community Mental Health Nurse to join the Central Locality Older Adult Community Mental Health Team.
The Central Locality Team screens incoming referrals from Primary Care and undertakes initial assessments. The team also provides ongoing support to Older Adults with dementia and complex enduring mental health issues. The team offers a range of management strategies and interventions where all care and support is offered in line with NICE Guidance.

Main duties of the job
Applicants must demonstrate a flexible approach evolving around the needs of patients, their families, and significant others. The role involves working as a team member, with individual responsibility of a mixed caseload of functional and organic cases.
We are looking for an enthusiastic person with excellent team working skills, who is keen to develop knowledge and skills in this area. You should be a competent and motivated practitioner with appropriate demonstrable experience. The successful applicant will work as part of a multi-disciplinary team providing assessment and therapeutic intervention in the community.
