Community Learning Disability Nurse

Job summary

An exciting opportunity has arisen to join the Community Learning Disability team within North Lincolnshire.

We are looking for a learning disability nurse who is progressive and dedicated to the delivery of services for people with learning disabilities.

The post holder will be based within the North Lincolnshire care group of Rotherham, Doncaster and South Humber Foundation Trust.

Main duties of the job

To undertake a range of assessments appropriate to the sphere of work, as designated by the Senior Community Nurse (Band 6).

To undertake interventions with clients as allocated by the Senior Community Nurse.

To manage a defined caseload, allocated by the Senior Community Nurse.

To assess and monitor clients physical and emotional well being and promote preventative healthcare within the client group.

To ensure access to mainstream health services, develop individual health action plans and promote health action facilitation.
