Job summary
An exciting opportunity has arisen for a Qualified Nurse or Allied Health Professional to join the Community Learning Disability team based in North Lincolnshire.
We are looking for a Clinical Lead who has effective leadership skills and can support and lead the team. They must be progressive and dedicated to the delivery of services for people with learning disabilities.
The post holder will be based within the North Lincolnshire care group working closely with the Service Manager and Team Manager to review the current business activities within the LD pathway. The post holder will also support in promoting the service, identifying good practice and working closely with the team to implement any recommendations.
The post will sit within the Intensive Support Service (ISS) as part of the wider Community Learning Disability multi-disciplinary team.
This team supports individuals with a primary diagnosis of a learning disability who may have also have a secondary diagnosis of autism spectrum disorder and/or a mental health condition.
Individuals may present with a risk to self, behaviours that challenge others, risk of placement breakdown or be experiencing/approaching periods of crisis.
Main duties of the job
Has responsibility for the promotion and assistance in the maintenance of the highest attainable standards of nursing care provided by the teams
To provide clinical leadership, guidance, support and coaching to staff, promoting effective change management and strengthening of other staffing leadership skills
Has the lead role of coordinating the team on a daily basis
Contribute to the management of complex case work
To plan, organise, undertake and document full bio-psychosocial assessment for patients referred to service
Ability to work closely with the team focusing on projects identified by management ensuring improvement of service and practice
The post holder will work closely with the Operational team Manager
