Job summary

We are looking for 3 enthusiastic B6 Occupational Therapists to work alongside the ever growing team of OTs, Physiotherapists, Advanced Nurse Practitioners and Rehab Assistants within the Doncaster Home First pathway.

The Home First pathway consists of Rapid Response, Virtual wards, Short-term care beds, Reablement and community Occupational Therapy and Physiotherapy services alongside the intermediate care beds.

The post holder will:

  • promote and improve the continuity of care from the acute hospital into the community setting
  • support patients in the community and ensure they have support at home to maintain and increase their independence
  • support patients who are in crisis who may require immediate intervention to reduce the need for them to be admitted to hospital
  • be personable with excellent communication skills
  • be expected to work alone and also inclusively with the multi-disciplinary team
  • be able to organise and prioritise own workload and balance this with the demands of this diverse and varied role

The Service operates over 7 days from 8am to 8pm.

We will provide support through in-house professional forums, Mandatory training, 1:1 and peer supervision. We facilitate student education and have close working relationships with neighbouring teams in both acute Health and Social Care.

You should be HCPC registered, have appropriate demonstrable post qualification community experience, have access to transport and be able to travel across the required geographical area.

Main duties of the job

The post holder will:

  • provide therapeutic assessment and intervention to patients within the Community, Hospice or Inpatient setting to aid discharge from hospital and avoid hospital admission
  • perform Occupational Therapy assessment of patients with various medical conditions in order to develop and deliver an individualised treatment program
  • provide managerial responsibility and developmental guidance to a Rotational Band 5 OT who is working within the service
  • provide supervision and developmental guidance to colleagues working within the team in relation to own areas of expertise and special interest, including nursing colleagues and Rehabilitation Assistant
  • devise and delegate work to Rehabilitation Assistants dependent on level of competence
  • work in partnership with the senior / leadership team to ensure on going service development and quality improvements to meet the needs of the changing client group
  • develop and deliver education packages for patients, carers, public and health care professionals on speciality area
  • work with primary, secondary care and nursing home staff and the multi-disciplinary team offering advice and educational input
  • engage with partner organisation regarding promoting the service and develop effective pathways for patients
  • support self and service development through active participation in continuing professional development (CPD) and by participating in any audit or research being undertaken by the service

  • 3-4 years clinical experience including at least 18 months working within community settings
