Launching the Primary Care Mental Health Hubs.

Doncaster is delivering a new place-based community mental health service for adults and older adults called the Primary Care Mental Health Hubs (PCMH). In the hubs you will get to work as part of a primary care mental health multidisciplinary team including; Clinical Psychologists, CBT Therapists, Clinical Associate Psychologists, Mental Health Wellbeing Practitioners, Primary Care Mental Health Practitioners, Occupational Therapists, Social workers, Community Connectors and Peer support workers. We are a partnership with RDASH, the local specialist Mental Health Trust, Primary Care Doncaster, and the Voluntary Community and Social Enterprise Sector (VCSE).
Mental Health transformation plans will mean the roll-out over the next three years of multi-disciplinary teams working within the new Primary Care Networks and offering wraparound, holistic care to our service users and wider community. We aim to provide personalised, trauma-informed care at a Primary Care Network level and are improving access to; evidence-based psychological interventions, valued activities in people's local communities, and medicine management.
Main duties of the job
The roles are part of the wider National Community MH Transformation programme which represents major change to enhance locality based mental health provision with new mental health roles being developed to grow the capacity within primary care. You will be joining this developing workforce at an important time and there will be real opportunities to be part of shaping this wider service delivery alongside statutory, anchor organisations and VCSE partners.

The new roles are fundamental todeveloping the innovative new teams and mental health services based within networks of GP Practices (PCN's). The roles will support the ongoing development and mobilisation of 4 integrated PCMH Hubs across Doncaster.
