Discharge Coordinator - Intermediate Care

Job summary

We have an exciting opportunity for a Discharge Coordinator in the intermediate care team who will be responsible for supporting complex discharges from East Sussex Healthcare NHS Trust. The purpose of the discharge coordinator role will be to identify patients at ward level via board rounds, multi-disciplinary team meetings and enhanced discharge meetings. The discharge coordinator will work in partnership with the ward based multi-disciplinary teams and the integrated discharge team, with the focus to reduce length of stay and facilitate early discharges.

The ESHT Rehabilitation strategy is focused on improving the quality and performance of our acute and community services to deliver high quality rehabilitation across East Sussex. This is a dynamic and changing environment, and the strategy is working to support our staff, teams and the organisation as a whole to make meaningful changes that bring about improvements in rehabilitation for the benefit of our population.

Main duties of the job

  • To work as part of the trust wide Intermediate Care Rehabilitation Service .

To work as part of a 7-day service over weekends and Bank Holidays on a rota basis

  • To hold a case load of non-complex cases under the supervision of a senior team member
  • To undertake specific support work to discharge processes from the intermediate care units.
  • To work unsupervised, reporting back on patient progress and escalation of any problems.
  • To assist in the induction of new staff within the ward area, supporting knowledge
