HSCC is a 24/7, 365 days a year, integrated frontline service, run by ESCC ASC department in conjunction with the local NHS Trusts. It is the first point of contact for members of the public with regards to social care needs and safeguarding, and also the main health professionals referral point
We are looking for a qualified nurse preferably with Community Nursing experience, to join our small, but busy, team of Clinical Nurse Advisors in HSCC, working various shifts to cover our core hours of 8am - 8pm, including weekends and Bank Holidays.
The successful candidate will have excellent communication skills and the ability to work both autonomously and alongside the larger HSCC team.
Main duties of the job
HSCC Clinical Nurse Advisors (CNA) support the operational management of District Nursing and Urgent Community Response referrals through applying clinical knowledge and experience and signposting the referrals accordingly.
We facilitate the provision of accurate healthcare information, self- care, health improvement advice, or where applicable, referral to the appropriate level and location of care, to meet the holistic and therapeutic requirements of the patient/carer.
