Faster Diagnosis Nurse Breast, and Head / Neck

Job summary

An opportunity has come up for a Breast and Head / Neck Fast Track Diagnostic Specialist Pathway Nurse.

This role will support these teams with reducing the backlog of patients on the Faster Diagnosis Pathway and deliver timely communication within 48 hours to patients identified on the 28 Day Patient Tracking List.

They will Liaise with the Multi disciplinary team to ensure patients are given results within the 28 day Urgent suspected cancer targets.

And that patients receive the optimum care.

Main duties of the job

Key duties and responsibilities

Deliver an improved reduction in the backlog of patients on the Faster Diagnosis Pathway and deliver timely communication within 48 hours to patients identified on the 28 Day Patient Tracking List.

Focus on achieving the 28 day standard for Breast and Head and Neck, assisting to expedite pathways.

  • To triage referrals into the service when required, using clinical expertise under the Breast Team and Head / Neck approval to correctly signpost to the next place in their pathway - identifying and escalating any incorrect
  • Liaise with GPs to discuss inappropriate/incomplete referrals.
  • To review current pathways with the Clinical Lead for 28 Day and 62 day treatment to reduce delays and to shorten pathways where clinically
  • To attend the weekly Multi-Disciplinary Meetings and with clinical agreement, contact patients to advise of the requirement of further diagnosis to avoid delays in corresponding with patients, thus expediting pathways to improve the patient experience and expedite treatment
  • Monitor and support pathways on a daily basis for complex patients often with co-morbidities to expedite appointments and undertake welfare calls and next steps to avoid additional
  • Discuss the individual patient case with appropriate personnel, g Surgeon, Oncologist, CNS, Diagnostic Team, Service Manager, PPC/Tracker etc. and work in close conjunction to support the patient pathway.
