Job summary

The Integrated Night Service is a 7 day night service that responds within 2 hours to the needs of patients in their own homes including residential care to avoid hospital admission.

Each shift consists of 3 teams based over the area (Bexhill, Eastbourne and Lewes), each team comprises of 1 trained nurse and 1 Health care assistant who conduct the visits jointly. Referrals from a range of sources (e.g. hospice, district nursing and 111) are handed to the team via health and social care connect. The HCA assist the nurses with their assessments, treatments and administration tasks.

We are looking for health care assists who ideally have experience working under the pressure of working outside of core hours. Excellent communication skills and a strong work ethic are required to enable high levels of patient care whilst adapting to the changing needs of the service.

Main duties of the job

The post holder will be an experienced Health care assistant who will work across the integrated night team services. We would encourage applications from health care assistants with knowledge and experience in a constantly evolving environment.

As a health care assistant in the integrated night team you will: Attend patients with a nurse to assist with rapid initial assessments and for follow up visits. Develop your skills into a more well-rounded holistic approach to patient care. There is also potential of some day shifts as the service expands. You will be expected to work weekends and bank holidays. Having assess to a car and being confident to drive at night is essential for this role.

The integrated night team cover the East Sussex patch which includes High Weald, Lewes and Havens, Eastbourne, Hailsham and Seaford and Hastings and Rother.
