Based at Arthur Blackman Clinic, St Leonards on Sea
This is an exciting and challenging post in which you will be part of a multi-disciplinary team delivering high-quality care to patients with Heart Failure.
Adopting a patient-centred approach you will support patients from their initial diagnosis through their medical management and optimisation. You will work collaboratively with other East Sussex Community Health Services Staff and East Sussex Healthcare NHS Trust and will require excellent communication skills.
You will manage your own caseload through Heart Failure nurse-led clinics and where necessary at home, providing personalised care plans in conjunction with primary care services. You will be involved in developing an effective, streamlined and efficient service for the patients whilst empowering them to be pro-active in managing their condition.

Essential
- BSc (Hons) or evidence of working towards.
- ENB 124/254 or equivalent cardiac course
- ENB 998/mentorship level 3 or equivalent teaching course
- Current RGN registration
- Evidence of continued professional development to Masters level or equivalent level of knowledge and experience
Desirable
- Masters Degree in Cardiology

- At least 5 years post registration experience including cardiology
- 3 years post registration at band 6 or above in a cardiac environment
- Experience of dealing with multidisciplinary teams including senior clinicians
