Job summary

Healthier South Wirral Primary Care Network (PCN) and CWP's Children Young People and Families mental health services (CYPMHS) have worked together to enhance access to mental health support.

We want to contribute to a more joined up approach for children, young people and families in South Wirral to facilitate getting the help and support in the right place for them, when they need it, to stay mentally well. Our aim is to deliver an integrated community-based model of mental health care supporting both primary and secondary care services.

Primary Care Networks (PCNs) are groups of GP practices working together in a local geographical area. Healthier South Wirral Care Network is a collaboration of 6 GP Practices (Sunlight Group Practice, Spital Surgery, The Orchard Surgery, The Allport Surgery, Eastham Group Practice, and Civic Medical Centre), in and around South Wirral who collectively care for over 49,000 patients.

We are looking for an experienced Mental Health Practitioner who is interested in working as part of the GP Multidisciplinary Team in Healthier South Wirral Primary Care Network. This is part of an exciting new development to improve collaborative working across primary and secondary care, the role will contribute to the NHS 10-year Plan, keeping care in the community, supporting transitions, navigating boundaries, and aligning with I-Thrive principles.

We are recruiting 2 x part time posts, each 0.70 fte (26.25 hours)

Main duties of the job

The role will include completing assessments, delivering brief interventions for patients registered at the GP practice who have mental health needs, or signposting to other provision in school or the community,supporting access to the graduated approach if there are potential neurodevelopmental needs, supporting signposting following assessment by secondary CYP Mental Health Services (CYPMHS), facilitating transitions out of CYPMHS and building community networks and offering training / upskilling. You will also provide expert advice and support to enable GP MDT to provide an effective service for children and young people with mental health needs.

You will be line managed and professionally supervised by the CWP Band 7 Access Team Manager. As the post is hosted via CWP you will also have access to training, education and support from CWP. This new post would suit individuals who enjoy working autonomously and who are committed to collaborative working to improve outcomes for people with mental health issues.

  • Registered Mental Health Professional, with professional qualification relating to Health (e.g. current registered RMN, Occupational Therapist)
