Job summary
An exciting and unique opportunity has arisen to be a part of the Community Learning Disability Team, Intensive Support Service based in Leasowe, on the Wirral. This service provides specialist assessment and intervention to help people with learning disabilities stay well and out of hospital. As an Assistant Practitioner you will be expected to support the Multi Disciplinary Team in providing intensive support to a small cohort of people with learning disabilities, providing high quality levels of care in order to assist these people to live fulfilled and independent lives in a community setting. You will be working within a forward thinking dynamic and supportive Multi-Disciplinary Team.
Main duties of the job
The post holder will be expected to work within a multi professionalframework, managing a defined case load of adult service users withLearning Disabilities and complex health needs, within the boundaries set out by the registered practitioner responsible for the service users care delivery.The post holder will be part of a team providing assessment and treatment to service users who may be in crisis, in a range of settings, and in partnership with other agencies, carers/family members and the wider multi- disciplinary team.The post holder will be expected to deliver training to small professionalgroups, service users and carers.The post holder will be expected to work flexibly in varying locations and atvarying times to meet the needs of the service
